Project greenBERRY Designer Entry Form

Please submit an entry form as soon as possible to be included in upcoming emails regarding Project greenBERRY. You can make changes to your entry once it has been submitted. The final date for entering a design is noon on Sunday, November 11, 2012. 

A. PERSONAL INFO
Primary Designer(s):  List up to two names. If you are submitting an entry from your club or organization, please submit one to two persons‘ names as primary designers. 

B. DESCRIBE YOUR GARMENT

Please write 100 to 125 words to describe your garment, accessories, and model. Be sure to describe (1) materials used, (2) what inspired you, (3) how you constructed your garment, and (4) be honest and tell us where you found your materials. Please include your model's name and school if applicable. This information will be shared as your model walks your design down the runway. After you submit your online entry form, you will receive and email with additional information and questions. If you need us to find a model to wear your design, please let us know as soon as possible.

Minors (designers and/or models) must be accompanied by parent or guardian from check in through end of event.  A release form must be signed at check in before participation.
