Forms for Semester Study Abroad
At your advising appointment you will be given the required forms for studying abroad for a semester. These include the
- Course approval form (must be submitted with course descriptions)
- Waiver of liability
We also require
- A Viking Web transcript (either emailed or hard copy brought to LAU 104)
- Recommendation from your academic adviser

This form needs to be notarized, and a list of notaries on campus is here. If you are not a Georgia resident, you can find a general power of attorney form for your state at an office supply store such as Office Depot. Make a copy of the completed form for your records and leave the original with the person to whom you have given power of attorney.
All forms are due February 15 for fall study abroad and September 15 for spring.
If your program does not include insurance, you are required to purchase study abroad insurance through iNEXT Insurance. You will receive an enrollment form at orientation.
