Involvement Information
Complete the categories below listing your most important leadership positions first. Each leadership position should be listed on its own line. Please do not repeat activities in multiple categories. Each response is limited to 250 characters.
School Related Organizations
This section involves any extracurricular activities through which you held a leadership role (involvement that extends beyond membership only). Examples include band, theater, clubs, sports, etc. Please include the organization name, the leadership position that you held, number of years involved, and any comments about the work you did while in the position.
Academic Honors and Awards
This section involves any special honors, awards, or recognition that you have received. Examples include Valedictorian, Governor’s Honors, Student of the Month, etc. Please include the award name, recipient year and a brief description of the award.
Volunteer and Service
This section involves any service through which you have taken a leadership role in a service organization or project outside of school organizations. Examples include Habitat for Humanity, Boys and Girls Club, Recycling Initiative, etc. Please list the name of the organization, the leadership role that you held, the number of years involved, and any additional comments about your work with that organization.

Community and Religious Activities
This section includes any leadership contribution you have had with a community or religious organization. Examples include Boy Scouts, Girl Scouts, Community Theater, Dance Troupe, Church Choir, Youth Group, etc. Please list the name of the organization, your leadership role, the number of years involved, and any additional comments about your work with the organization.
Work Experience
This section includes your work experience throughout your high school career. Examples include restaurant server, office assistant, child care provider, lawn maintenance, etc. Please list the name of your employer, your job title, the number of years you held the position and any additional comments about your leadership within that job.
Leadership Reflections
Complete the following leadership reflections. Each question allows for a maximum of 1000 characters.
Question 1 - What would you identify as your leadership strengths? In what areas would you like to improve?
Question 2 - Consider one of the activities you listed above. In what way did you show initiative as a leader? How did this involvement challenge you or help you grow as a leader?
Question 3 - Leaders often exhibit passion for an issue or need that they want to address in their school or community. Examples include environment, social justice issues, school spirit, diversity, tolerance, etc. Describe an issue or need that you are passionate about and discuss how you would use your leadership skills to improve this area.
